Stopping Wegovy

Wegovy is semaglutide at the 2.4 mg weight-management dose, the same molecule as Ozempic. It is approved for chronic weight management and to lower the risk of heart attack and stroke in adults with established cardiovascular disease and obesity or overweight. The semaglutide withdrawal trials were run at exactly this dose.

What to expect week by week

With a half-life of about 1 week, Wegovy does not switch off the day you stop. The share of drug left, estimated from the half-life alone:

  • First 1–2 weeks: little change for most people. Nausea and other gastrointestinal side effects usually start to ease.
  • Weeks 2–5: appetite, portion size and "food noise" typically come back as levels fall, and larger meals feel comfortable again.
  • From week 6: the drug is largely cleared. What happens to the weight from here depends on the habits in place.

Weight regain after stopping semaglutide

STEP 1 extension

After 68 weeks of semaglutide 2.4 mg, treatment stopped and participants were followed for another year with no drug and no structured lifestyle program.

Within a year of stopping, participants had regained about two-thirds of the weight they had lost. Improvements in blood pressure, blood sugar and cholesterol drifted back toward where they started.

Trial summary

STEP 4

Everyone took semaglutide 2.4 mg for 20 weeks and lost about 10.6% of body weight. Participants were then randomized to keep taking it or switch to placebo for 48 weeks.

Those who continued lost a further 7.9%. Those switched to placebo regained 6.9% of body weight over the same 48 weeks.

If you take Wegovy partly for your heart

The cardiovascular benefit shown in the SELECT trial was measured in people who kept taking semaglutide. If heart protection is part of why you were prescribed Wegovy, raise it with your cardiologist or prescriber before stopping.

Wegovy taper schedule

The Wegovy dose ladder (0.25 mg, 0.5 mg, 1 mg, 1.7 mg, 2.4 mg) in reverse, about 4 weeks per dose. There is no official taper protocol, and tapering has not been shown to reduce regain in a randomized trial. Take this to your prescriber as a starting point, not a prescription.

Stopping Wegovy: common questions

How long does Wegovy stay in your system?

Wegovy (semaglutide) has a half-life of about 1 week, so it takes around 5 weeks after the last injection for it to clear. Levels fall by about half every week, which is why many people notice appetite returning a couple of weeks after stopping, before the drug is fully gone.

Do you need to taper off Wegovy?

No official taper exists, and stopping outright is not dangerous for most people. A step-down, running the Wegovy dose ladder in reverse at about 4 weeks per dose, slows the return of appetite and can show whether a lower dose holds your weight. Tapering has not been proven to reduce regain in a randomized trial. Decide with your prescriber; the Taper Planner lists a gradual and a shorter step-down from every Wegovy dose.

Will I gain weight after stopping Wegovy?

Most people in the semaglutide withdrawal trials regained a large share of the weight within a year. In the STEP 1 extension, people regained about two-thirds of the weight they had lost within a year of stopping semaglutide. In STEP 4, people switched from semaglutide to placebo regained 6.9% of body weight over 48 weeks, while those who continued lost a further 7.9%. Results varied widely, and participants stopped with no structured plan. Protein, strength training and an early response to a rising trend are the parts you control.

What happens if you stop Wegovy cold turkey?

There is no recognized withdrawal syndrome. Gastrointestinal side effects such as nausea usually ease within a few weeks, and appetite and "food noise" typically return as levels fall over about 5 weeks.

Can I restart Wegovy after stopping?

Usually, if your prescriber and coverage allow it. After a gap of more than a couple of weeks, prescribers normally restart at a lower dose and step back up to limit nausea and other side effects. Agree the restart rule, meaning which weight trend triggers it and at what dose, before you stop.

Can I switch to Ozempic instead of stopping Wegovy?

Wegovy and Ozempic contain the same molecule (semaglutide); Ozempic is approved for type 2 diabetes rather than weight management, so coverage rules usually decide which one you can get. If cost or coverage is why you are stopping, ask your prescriber whether a switch, a lower dose or another option makes sense. The dose ladders differ, so a switch needs a new dosing plan.
