Side Effects of Stopping Ozempic, Wegovy or Zepbound β€” and Is There Withdrawal?

Stopping a GLP-1 drug does not cause withdrawal in the way stopping an antidepressant, a beta-blocker or a sleeping pill can. Semaglutide and tirzepatide are not habit-forming, and there is no recognized rebound syndrome. What people describe as "Ozempic withdrawal" is the drug's effects wearing off, and most of those effects are the reasons they were taking it in the first place.

What changes after the last dose

Both drugs last days in the body, so nothing switches off overnight. Semaglutide (Ozempic, Wegovy) has a half-life of about a week and takes around five weeks to clear. Tirzepatide (Zepbound, Mounjaro) has a half-life of about five days and clears in around four weeks.

The side effects of stopping

Hunger and food noise come back

This is the change people notice most. GLP-1 drugs slow stomach emptying and act on appetite centers in the brain, so as levels fall, meals feel less filling and thoughts about food return. Weight comes back for most people

In the randomized withdrawal trials, most people regained a large share of the weight within a year of stopping, though on average not all of it. The details are in do you gain weight after stopping a GLP-1?

Blood sugar rises in type 2 diabetes

Ozempic and Mounjaro are diabetes medicines first. They lower blood sugar by boosting insulin release when glucose is high and damping glucagon. When they clear, HbA1c usually rises unless another treatment takes over. If you use insulin or a sulfonylurea, the doses may need adjusting. This is the one effect of stopping that needs a plan before the last dose, not after.

Blood pressure and cholesterol drift back

In the semaglutide trials, improvements in blood pressure and cholesterol moved back toward where people started as the weight returned. If these numbers improved while you were on the drug, ask when they should be rechecked.

The stomach side effects fade

The good news: nausea, vomiting, constipation, reflux and early fullness usually settle within a few weeks of stopping. The catch is that the same change makes bigger portions comfortable again, which is part of why weight comes back.

Mood

Some people describe feeling anxious or low after stopping, often around returning hunger or the fear of regaining. There is no good evidence that stopping a GLP-1 causes a mood disorder directly, but the experience is real and worth planning for. If low mood persists, talk to your clinician. Stopping cold turkey vs tapering

For most people stopping suddenly is not dangerous, and because both drugs have long half-lives, levels already fall gradually over four to five weeks. Tapering, stepping down through lower doses before stopping, makes the return of appetite slower still and can show whether a lower dose holds your weight. It has not been shown in a randomized trial to reduce regain.

When to call your prescriber

  • Your glucose readings are above your target, or you notice thirst, frequent urination or blurred vision (type 2 diabetes).
  • Your 7-day average weight crosses the threshold you agreed before stopping.
  • You want to restart. After a break of more than a couple of weeks, restarting at your old dose can bring the nausea back hard, so prescribers usually restart lower and step back up.
  • Your mood is low for more than a couple of weeks.

Is there such a thing as Ozempic withdrawal?

Not in the medical sense. There is no dependence and no rebound syndrome. The term usually describes hunger, food noise and weight returning as semaglutide clears over about five weeks.

What happens if you stop Ozempic suddenly?

Levels fall gradually over several weeks anyway. Stomach side effects ease, appetite returns, and in type 2 diabetes blood sugar can rise, which is why stopping should be planned with your prescriber.

Do side effects get worse when you stop Zepbound?
