Calcitonin gene-related peptide (CGRP) is a 37-amino-acid neuropeptide produced by alternative splicing of the calcitonin gene (CALCA). It exists in two isoforms: alpha-CGRP (predominantly expressed in sensory neurons of the dorsal root and trigeminal ganglia) and beta-CGRP (expressed primarily in enteric neurons). CGRP is the most potent endogenous vasodilator identified, approximately 1000-fold more potent than acetylcholine and 10-fold more potent than prostaglandins on a molar basis. It is released from sensory nerve endings during neurogenic inflammation and acts through the CGRP receptor complex — a heterodimer of the calcitonin receptor-like receptor (CLR) and receptor activity-modifying protein 1 (RAMP1). CGRP achieved clinical prominence as a central mediator of migraine pathophysiology. During migraine attacks, CGRP levels are markedly elevated in jugular venous blood and trigeminal ganglia, and exogenous CGRP infusion can trigger migraine-like headaches in susceptible individuals. This understanding led to development of anti-CGRP monoclonal antibodies (erenumab, fremanezumab, galcanezumab) and small-molecule CGRP receptor antagonists (gepants) that have transformed migraine prevention and treatment. However, CGRP is not solely a pain mediator — it plays protective roles in wound healing, cardiovascular homeostasis, ischemic preconditioning, and immune modulation. This dual nature as both a migraine trigger and a tissue-protective molecule underscores the complexity of neuropeptide biology.
