Dalcinonacog alfa is a recombinant human coagulation factor IX glycoprotein produced in genetically engineered Chinese hamster ovary (CHO) cells. It is structurally and functionally analogous to endogenous factor IX, a vitamin K-dependent serine protease essential for normal blood coagulation via the intrinsic pathway. When activated to factor IXa, it forms a complex with activated factor VIII (tenase complex) on phospholipid surfaces in the presence of calcium ions, catalyzing the conversion of factor X to factor Xa — a critical step in thrombin generation and subsequent fibrin clot formation. The recombinant manufacturing process eliminates risks associated with plasma-derived products, including potential transmission of blood-borne pathogens. Clinical development of dalcinonacog alfa demonstrated robust hemostatic efficacy across multiple pivotal trials involving previously treated patients with moderate to severe hemophilia B. In the Phase III study, the product achieved an overall hemostatic efficacy rating of 'excellent' or 'good' in over 95% of bleeding episodes when used for on-demand treatment, with most bleeds controlled with a single infusion. Prophylactic regimens showed significant reductions in annualized bleeding rates compared to on-demand treatment alone. The pharmacokinetic profile revealed dose-proportional increases in factor IX activity with a mean terminal half-life of approximately 22–25 hours, supporting twice-weekly prophylactic dosing. The safety profile was favorable, with low immunogenicity — no patients in clinical trials developed neutralizing inhibitors (anti-factor IX antibodies) — and the most common adverse reactions were headache, dizziness, and injection-site reactions.
