Daptomycin is a 13-amino-acid cyclic lipopeptide antibiotic derived from Streptomyces roseosporus, approved by the FDA in 2003 (marketed as Cubicin) for the treatment of complicated skin and soft tissue infections and Staphylococcus aureus bacteremia, including right-sided endocarditis. It was the first member of its antibiotic class to reach clinical use and remains the only cyclic lipopeptide in clinical practice. Daptomycin's mechanism is fundamentally distinct from all other antibiotic classes: in the presence of physiological calcium concentrations, the peptide's lipophilic decanoyl tail inserts into the Gram-positive bacterial cell membrane, causing the molecule to oligomerize and form ion-conducting channels that rapidly depolarize the membrane potential. This depolarization halts DNA, RNA, and protein synthesis simultaneously, producing rapid bactericidal activity without cell lysis — a characteristic that may reduce the inflammatory surge associated with lytic antibiotics. The calcium-dependent mechanism gives daptomycin several clinically important properties. It is exclusively active against Gram-positive organisms because the outer membrane of Gram-negative bacteria prevents access to the cytoplasmic membrane target. Its bactericidal activity is concentration-dependent rather than time-dependent, supporting once-daily dosing that achieves high peak-to-MIC ratios. Daptomycin retains full activity against methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-resistant enterococci (VRE), and most other drug-resistant Gram-positive pathogens, making it a critical reserve antibiotic in the era of antimicrobial resistance. However, daptomycin is inactivated by pulmonary surfactant, rendering it ineffective for pneumonia — a significant clinical limitation that restricts its use to non-pulmonary infections. Administration is exclusively intravenous; the peptide's size and charge prevent oral absorption. Standard dosing is 4–6 mg/kg once daily for skin infections and 6–10 mg/kg for bacteremia, with higher doses (8–12 mg/kg) increasingly used in clinical practice for deep-seated infections based on pharmacokinetic-pharmacodynamic optimization data. The primary adverse effect requiring monitoring is creatine phosphokinase (CPK) elevation, reflecting potential skeletal muscle toxicity, which occurs in approximately 2–7% of patients and is typically reversible upon discontinuation. Weekly CPK monitoring is recommended during therapy. Eosinophilic pneumonia is a rare but recognized idiosyncratic reaction. Daptomycin resistance, while still uncommon, has been documented — primarily through mutations in the mprF gene that alter cell membrane phospholipid composition, reducing daptomycin binding efficiency.
