Efpeglenatide is a long-acting GLP-1 receptor agonist distinguished by its molecular design. Unlike the human-GLP-1-based agonists (semaglutide, liraglutide), efpeglenatide is derived from exendin-4 — the same lizard-venom peptide that inspired exenatide — and is fused to a modified immunoglobulin Fc fragment. This Fc-fusion 'LAPS' (Long-Acting Protein/peptide discovery platform) technology dramatically extends the circulating half-life, enabling once-weekly and potentially less-frequent dosing. The exendin backbone makes efpeglenatide resistant to DPP-4 degradation and gives it high potency at the GLP-1 receptor. Functionally it behaves like other GLP-1 agonists: it enhances glucose-dependent insulin secretion, suppresses postprandial glucagon, slows gastric emptying, and reduces appetite and body weight. Efpeglenatide's most significant contribution to the field is the AMPLITUDE-O trial — a large cardiovascular outcomes study in people with type 2 diabetes and either established cardiovascular disease or kidney disease with additional risk factors. The trial reported that efpeglenatide reduced the risk of major adverse cardiovascular events (MACE) and also showed benefit on composite kidney outcomes. This was an important result because it extended the cardioprotective GLP-1 story to an exendin-based, Fc-fusion agonist and to a population enriched for chronic kidney disease. Despite the positive outcomes data, efpeglenatide's commercial development has been complicated, and it is not broadly approved. It remains primarily of interest as a research and reference compound within the GLP-1 landscape.
