Gallium Ga-68 dotatate is a diagnostic radiopharmaceutical — a somatostatin-analog peptide (DOTATATE) labeled with gallium-68, a positron-emitting radionuclide used for PET imaging. The peptide binds somatostatin receptors, particularly subtype 2 (SSTR2), which many neuroendocrine tumors (NETs) over-express. After a small tracer dose is injected intravenously, the peptide concentrates wherever these receptors are abundant, and a PET/CT scanner detects the gallium-68 signal to produce a map showing the location and extent of receptor-positive tumor tissue throughout the body. This makes gallium-68 dotatate PET/CT a highly sensitive tool for finding neuroendocrine tumors and their metastases, which can otherwise be hard to detect. It has largely superseded older somatostatin-receptor imaging (such as indium-111 pentetreotide/OctreoScan) for many indications because of its superior image quality and sensitivity. It is a diagnostic tracer, not a treatment: it carries only a small amount of radioactivity intended for imaging, not a therapeutic radiation dose. Crucially, gallium-68 dotatate plays a 'theranostic' role — pairing diagnosis with therapy. Because it uses the same somatostatin-analog targeting as therapeutic agents like lutetium Lu-177 dotatate (Lutathera), a positive scan both diagnoses the tumor and predicts whether it is likely to respond to somatostatin-analog treatment or PRRT: if the tumor takes up the diagnostic peptide, it should also take up the matched therapeutic peptide. Gallium Ga-68 dotatate (NETSPOT in the US; SomaKit TOC in Europe) is an approved imaging agent administered in nuclear-medicine settings.
