Glatiramer acetate is unlike almost any other peptide medicine: it is not a single defined molecule but a controlled mixture of synthetic peptide chains built at random from four amino acids — L-glutamic acid, L-lysine, L-alanine, and L-tyrosine — in a fixed molar ratio and a defined size range. It was originally designed decades ago to mimic myelin basic protein, a component of the myelin sheath that insulates nerves and is a target of the misdirected immune attack in multiple sclerosis. Rather than mimicking the disease, it turned out to modulate the immune response against myelin. Its approved use is relapsing forms of multiple sclerosis (MS), where the immune system attacks the central nervous system's myelin, causing relapses and accumulating disability. The precise mechanism is not fully defined, but glatiramer acetate is thought to shift the immune response away from inflammatory, myelin-attacking T cells toward regulatory, anti-inflammatory ones, and to induce 'bystander' immune cells that dampen inflammation in the nervous system. Whatever the exact pathway, large trials established that it reduces relapse frequency in relapsing-remitting MS, and it has one of the longest real-world safety records of any MS disease-modifying therapy. Glatiramer acetate is given by subcutaneous injection, either daily (20 mg) or three times weekly (40 mg), and is available both as the original branded product and as approved generics. It is generally well tolerated, without the immunosuppression-related infection risks of many newer MS drugs, which is part of its enduring appeal — especially where a well-characterized safety profile is a priority. Its most distinctive side effect is a transient post-injection reaction (flushing, chest tightness, palpitations, and anxiety lasting minutes) that is alarming but self-limited and not dangerous, along with injection-site reactions. It is a prescription MS therapy used under neurology care.
