Gonadorelin acetate is the acetate salt of synthetic gonadotropin-releasing hormone (GnRH), a decapeptide originally isolated from porcine and ovine hypothalami by Andrew Schally and Roger Guillemin, work that earned the 1977 Nobel Prize in Physiology or Medicine. The sequence pyroGlu-His-Trp-Ser-Tyr-Gly-Leu-Arg-Pro-Gly-NH2 is identical across all mammals studied, underscoring its evolutionary conservation. When administered as a pulsatile bolus, gonadorelin binds the GnRH receptor (GnRHR) on anterior pituitary gonadotrophs, triggering phospholipase C/IP3/DAG signaling that results in rapid exocytosis of luteinizing hormone (LH) and, to a lesser extent, follicle-stimulating hormone (FSH). This pulsatile release pattern is critical — continuous GnRH exposure paradoxically downregulates the receptor, a principle exploited by long-acting GnRH agonists such as leuprolide for chemical castration. Clinically, gonadorelin has FDA-approved diagnostic uses: the GnRH stimulation test assesses pituitary gonadotroph reserve in suspected hypogonadotropic hypogonadism, delayed puberty, and hypothalamic amenorrhea. Therapeutically, pulsatile gonadorelin delivered via portable infusion pumps has been used to restore fertility in patients with hypothalamic amenorrhea — the pulsatile delivery mimics the endogenous GnRH pulse generator and can restore normal menstrual cyclicity and ovulation. In the peptide therapy community, gonadorelin is also used off-label as a post-cycle therapy agent to stimulate endogenous testosterone production after exogenous androgen use, leveraging its ability to acutely release LH from the pituitary. Its very short half-life (2-4 minutes intravenous) necessitates either subcutaneous bolus dosing for acute tests or pulsatile pump delivery for sustained therapeutic effect.
