Insulin aspart is a rapid-acting analog insulin in which a single amino acid on the B-chain (proline at position B28) is replaced by aspartic acid. As with insulin lispro, this change does not alter the hormone's action at the receptor, but the negatively charged aspartic acid reduces the tendency of insulin molecules to associate into hexamers, so the drug dissociates into absorbable monomers faster after a subcutaneous injection. The effect is a mealtime insulin with a rapid onset (about 15 minutes) and a short duration that can be dosed right around meals, matching the glucose surge from eating more closely than human Regular insulin. Insulin aspart is a mainstay bolus insulin in basal-bolus regimens and in insulin pumps for type 1 and type 2 diabetes. An ultra-rapid formulation, Fiasp, adds niacinamide and other excipients to accelerate early absorption, allowing dosing at the start of a meal or shortly after. Insulin aspart is a prescription medicine, and like every insulin its principal risk is hypoglycemia; because it acts quickly, coordination with food intake matters. It provides mealtime coverage only and must be combined with a basal insulin (or a pump's basal delivery) for full 24-hour control. It is used under medical supervision and is not a performance-enhancing or wellness product.
