Lixisenatide

Lixisenatide is a synthetic 44-amino-acid peptide and potent glucagon-like peptide-1 (GLP-1) receptor agonist based on the exendin-4 backbone originally isolated from the saliva of the Gila monster (Heloderma suspectum). The molecule features key structural modifications compared to native exendin-4: deletion of a proline residue and addition of six lysine residues at the C-terminus, which enhance receptor binding affinity and confer resistance to dipeptidyl peptidase-4 (DPP-4) enzymatic degradation. Upon subcutaneous injection, lixisenatide binds to and activates the GLP-1 receptor on pancreatic beta cells, stimulating glucose-dependent insulin secretion while simultaneously suppressing inappropriate glucagon release from alpha cells. A distinguishing pharmacological feature of lixisenatide is its pronounced effect on postprandial glucose excursions, achieved through marked slowing of gastric emptying — a property that differentiates it from longer-acting GLP-1 receptor agonists that exert more balanced fasting and postprandial glucose control. The clinical efficacy of lixisenatide was established through the comprehensive GetGoal Phase III trial program, which included studies of lixisenatide as monotherapy (GetGoal-Mono), in combination with basal insulin (GetGoal-L, GetGoal-Duo 1), and as add-on to oral antidiabetic agents (GetGoal-P). These trials demonstrated significant HbA1c reductions of 0.7–1.0% and robust postprandial glucose lowering. The landmark ELIXA cardiovascular outcomes trial, involving 6,068 patients with type 2 diabetes and recent acute coronary syndrome, demonstrated cardiovascular safety with non-inferiority to placebo for major adverse cardiovascular events (MACE). Lixisenatide received FDA approval in 2016 and EMA approval in 2013. The fixed-ratio combination product Soliqua 100/33 (insulin glargine/lixisenatide) was subsequently approved in 2016, providing a simplified once-daily regimen that addresses both fasting and postprandial glycemic targets.
