Cortexin is a polypeptide preparation obtained by acid extraction and enzymatic processing of bovine cerebral cortex tissue. The resulting lyophilized product contains a mixture of low-molecular-weight neuropeptides (typically under 10 kDa), free amino acids, vitamins, and trace minerals. It was developed at the Institute of Bioregulation and Gerontology in Saint Petersburg and has been registered as a pharmaceutical product in Russia since 1999, manufactured primarily by Geropharm. The preparation is classified as a nootropic and neuroprotector in Russian pharmacological nomenclature. It is used clinically across Russia and CIS countries for a range of neurological conditions including acute ischemic stroke, traumatic brain injury, cognitive impairment in elderly patients, epilepsy (as adjunct therapy), and neurodevelopmental disorders in children. Russian clinical literature reports improvements in cognitive function scores, EEG normalization, and accelerated neurological recovery in multiple patient populations. Cortexin is frequently compared to Cerebrolysin, a porcine brain-derived peptide preparation manufactured by EVER Neuro Pharma (Austria). Both are multi-peptide mixtures with neurotrophic activity, but they differ in source species (bovine vs. porcine), manufacturing process, route of administration (Cortexin is IM only; Cerebrolysin can be IV), and geographic market presence. Cerebrolysin has larger international RCTs (including the CASTA trial), while Cortexin's clinical data is predominantly from Russian-language publications. Western regulatory agencies have not evaluated or approved Cortexin. Its clinical evidence base, while extensive in Russian medical literature, has not been independently replicated in Western trial settings. The preparation remains available primarily through Russian pharmacies and international peptide suppliers.
