Telavancin is a semisynthetic lipoglycopeptide antibiotic derived from vancomycin, designed with a lipophilic side chain and additional chemistry that give it a dual mechanism of action. Like the parent glycopeptide, it binds the D-alanyl-D-alanine terminus of peptidoglycan precursors to inhibit cell-wall synthesis; in addition, its membrane-anchoring tail disrupts the bacterial cell membrane, depolarizing it and increasing permeability. This two-pronged activity produces rapid, concentration-dependent killing of Gram-positive bacteria, including methicillin-resistant Staphylococcus aureus (MRSA). Telavancin is dosed once daily by intravenous infusion, adjusted for kidney function. It is approved for complicated skin and skin-structure infections and for hospital-acquired and ventilator-associated bacterial pneumonia caused by susceptible Staphylococcus aureus when alternative treatments are not suitable. Its spectrum is Gram-positive. Telavancin (Vibativ) is an approved prescription antibiotic, but it carries important safety considerations that shape its use. It has boxed warnings for increased mortality seen in certain patients with pre-existing moderate-to-severe kidney impairment treated for hospital-acquired/ventilator-associated pneumonia, for nephrotoxicity (new or worsening kidney impairment), and for fetal risk (a pregnancy status must be confirmed before use, with a pregnancy registry). It can also interfere with some coagulation laboratory tests. For these reasons telavancin is generally reserved for situations where other therapies are not appropriate, and it is used strictly under specialist and stewardship guidance rather than as a routine first choice. It is a clinician-administered medicine, not a research or consumer peptide.
