Thymopentin (TP-5) is a synthetic pentapeptide with the sequence Arg-Lys-Asp-Val-Tyr, representing the minimal active fragment (residues 32–36) of thymopoietin, a 49-amino-acid hormone produced by thymic epithelial cells. Thymopoietin plays a central role in T-cell differentiation and maturation within the thymus, and thymopentin retains the full immunomodulatory activity of the parent hormone in a more stable and pharmaceutically practical form. It induces differentiation of T-cell precursors, enhances mature T-cell function, and modulates the balance between T-helper and T-suppressor cell populations. Thymopentin has been approved and used clinically in several countries — notably Italy, China, and parts of Asia — for conditions including primary immunodeficiency, rheumatoid arthritis, chronic hepatitis B, recurrent respiratory infections, and as an adjuvant in cancer immunotherapy. Clinical trials conducted primarily in the 1980s and 1990s demonstrated improvements in T-cell counts, clinical symptom scores, and infection frequency in immunocompromised patients. While it has not been approved by the FDA or EMA, thymopentin represents one of the more clinically validated thymic peptides, with a larger body of human data than most peptides in this category.
